KDPL Data Subject Request Form

This is the official application form to exercise your rights under the Personal Data Protection Law No. 6698 (KDPL). Applications are free of charge; per KDPL art. 13/2 you will receive a written response within 30 days.


§1 Process

Five steps to apply

    Download the form

    Download the PDF to your computer (printable, A4 portrait, 2 pages).

  2. 2

    Fill it in

    Enter your personal details, tick the rights you wish to exercise, and describe your request.

  3. 3

    Sign by hand

    Complete the date and signature fields by hand. If you are a legal entity, attach the signature circular.

  4. 4

    Submit it

    Send it to us through one of the channels below (post / KEP / in person).

  5. 5

    Wait 30 days

    Per art. 13/2 of the KDPL we deliver a written response within 30 days.

B — Subject of Application (KDPL art. 11)

Tick the data-subject right(s) you wish to exercise:

  • To learn whether my personal data is being processed
  • To learn the purpose of processing and whether the data is used in line with that purpose
  • To learn the third parties — domestic or abroad — to whom my data has been transferred
  • To request rectification of my personal data that is incomplete or inaccurate
  • To request erasure or destruction of my personal data (KDPL art. 7)
  • To request that any rectification or erasure be notified to third parties to whom the data has been transferred
  • To object to decisions reached against me through automated processing systems
  • To claim compensation for damages suffered due to unlawful processing under the KDPL

A handwritten (wet-ink) signature is required — an electronic signature is not accepted. For legal-entity applications, a signature circular must also be attached.


§4 After Application

Response time and right to complain

  • Once your application reaches us, we acknowledge receipt within 24 hours.
  • Per KDPL art. 13/2, a written response is provided within 30 days; if that deadline is missed, you may complain to the Authority.
  • The application is free of charge; if your request requires a specific cost (printed copies, etc.), the Board's tariff applies.
  • If you are not satisfied with the response, your right to file a complaint with the Personal Data Protection Board (kvkk.gov.tr) is reserved.
